First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. We will make every effort to complete your first cleaning or restorative visit along with your consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment and begin treatment on another day.  Also there are procedures that require more than one visit to complete. If that situation applies to your treatment plan we will discuss those procedures and necessary appointments at the time of your initial visit.

Enclosed in this website you will find information forms that need to be filled out in their entirety.  Please remember to bring them to your appointment.  Also, please be sure to read and sign the bottom/back side of the patient information sheet.  When you come for your appointment you will also need to bring:

• Current X-rays from your previous dentist (if applicable) or make arrangements for the x-rays to be sent to our office in time for your appointment.

• Current Insurance Card(s).  If no card is available, you will need to bring the insurance company's address/phone/group number and identification number.  Without this information we will not be able to process your insurance claim. Your benefits booklet is helpful in determining your portion.  If you do not have a booklet you can contact your Human Resources Department for a copy to bring with you.

• Please be prepared at the time of service to pay your coinsurance/deductibles and fees not covered by or through an insurance plan. We accept MasterCard, Visa, Discover, Personal Check and Cash. We understand that some larger treatment plans are difficult to pay at the time of your visit.  For services over $300.00 we offer extended payments through the CareCredit Personal Finance program.  See our financial page for a link and additional information.
• Please alert the office if you have a medical condition that may be of concern at the time you set your appointment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require taking medication prior to dental cleanings (i.e antibiotics, for pre-med.)  You will also need to bring a list of all medications you take including herbal remedies.
